PAN / VAT Number: Your 9-digit Permanent Account Number as issued by IRD Nepal.

Tender Reference: The exact tender number and title (e.g. KHP/NCB/004/082-83 — Supply of Road Construction Materials).

Submission Reference ID: The Reference ID received after bid submission (e.g. KHP/NCB/004/082-83-024A2958).

Statement of Withdrawal: "We hereby officially withdraw our sealed bid referenced above."

Date: Date the letter is signed. Bikram Sambat (BS) or AD format are both accepted.

Authorised Signatory: Full name, designation, and wet-ink signature of the authorized representative.

Company Seal: Official company rubber stamp / seal (mandatory under the Public Procurement Act).
